I had a similar awkward conversation with a doctor two weeks ago about a possible UTI and the likelihood of it having been sexually transmitted (i.e., "It's really, really outlandishly unlikely that I've gotten an STI from anything I've done since the last time I was tested"). He ended up insisting on doing an STI test anyway, which I finally agreed to just so I wouldn't have to go into any more detail about my sex life.
